My name is Shelly Hovick and I am a PhD student at the University of Georgia. I am seeking participants for a research study on family health information.

As you and your fiancé prepare for your life together, the topic of health has probably come up. You might have shared information about your own health or about illnesses that run in your families. This can be important information, as health problems that run in your family may increase your own risk. But, have you ever stopped to think about how much you really know about your family's health?

I hope you and your fiancé will each take this survey! The survey assesses what you know about your family's health and your willingness to find out more information. You can also enter to win a $100 gift certificate from Bed Bath and Beyond. Participation in the survey not required to enter the drawing.
